Participant, Parents and Guardians understand that the agreement to participate in any event at Evolve Martial Arts acknowledges that participants will be engaged in physical activities with other children, participants, guests, staff and instructors. All exercises and activities, including the use of equipment, and facility shall be at the participant’s sole risk. Participants may encounter liquid or equipment on the floor, walls, and other objects that can cause injury. Participate is held accountable to avoid such objects and or conditions. Parents, Guardians, and Participants shall not hold liable the School, its officers, owners, agents, students, guests, other participants, staff and employees, all claims which may be brought against by injuries that many occur. Participants will use their own medical coverage and be responsible for any deductibles that may arise from any injury or incident. All physical martial arts activities run the risk of falling, running into other objects and participants. Participants will be motivated and encouraged to step outside of their comfort zone in order to learn lessons and experience success. Participants that are not following indirections and/or are causing a distraction, safety issue, or are deviant in behavior may result in having the participant “take a break” until they are ready to rejoin the group. If a participant damages any property or equipment in the school, due to misuse, carelessness, or misbehavior, the Parent or Guardian who signed this form may be held liable to repair or replace the damages. Parent or Guardian agrees and releases all rights to any pictures and videos that may be taken during class or an event.
